Workspaces can send patients an ACC new injury form to complete which will populate the patients ACC patient details and ACC claim details in splose.

These forms can be sent in two ways:

You can change the theme colour, form header image and form completion message in ACC form settings.


Method 1: Manually via the patient's profile

In a patient's profile, navigate to ACC details.

Scroll down to ACC forms.

Email ACC form to patient

To email an ACC form to a patient, click + Add new form.

Give the form a title, such as ACC form.

Click on the three dots.


Click email form, compose your email and send.

  • Note, that you can create an ACC form email template within the email templates section, allowing you to streamline the process and avoid composing the email from scratch each time.


Method 2: Automatically send an ACC form

You can automatically send an ACC form to a patient when an appointment is booked using appointment templates.

Navigate to settings > appointment templates.

Edit or create a confirmation and/or reminder appointment template.

Add in an ACC form link placeholder.

Reviewing completed form details

Once the form is complete, the patient's ACC patient details and new draft ACC claim will be both be ready for review from within the ACC detail section of their profile.

To complete the new draft ACC claim navigate to the ACC tab. You will see a list of all ACC claims which you can filter by Draft status. Click on the three dots under the Actions column to edit the form and complete the practitioner details such as adding a read code.


If the patient has ticked that they have an existing ACC claim then you will need to check the completed form for the claim number that they have provided. You can then copy this into the import claim feature - read more here.


ACC form settings

Under settings, you can change the theme colour, form header image and form completion message.

You can also choose to automatically email the patient a copy of the completed form and notify users or user groups in your workspace that the form has been completed.
