Track who submitted a form

Easily track who has completed forms submitted to your practice, and on whose behalf.

splose can record who completed a form: the client, or someone filling it in for them, such as a parent or referrer. Before the form is submitted, a Before you submit step asks the person to confirm who they are. splose then saves their answer in the form's change log, which can't be edited or deleted.

This gives you a reliable record if a signed form, such as a service agreement, is ever questioned, for example during an audit, a funder’s review of your records, or a complaint. It works for both standard and embeddable forms.

Turn the 'Before you submit' section on or off

To enable the Before you submit section in a form:

  1. Go to Settings, then under Automation click Forms.

  2. Click the three dots in the Actions column next to the form, then select View.

  3. Click Settings in the top-right corner.

  4. Under Form completion, tick or untick Ask who is completing this form before it's submitted.

  5. Click Save.

⚠️ The step is on by default for new form templates and off for templates created before this feature was released. To use it on an existing form, turn it on in that form's settings.

What the client sees

When the step is on, a Before you submit section appears at the end of the form. The person filling it in must answer before they can submit the form. This includes practice staff filling in a form for a client, for example during a session. They choose Practice staff as their relationship.

  • If they're the client, they confirm the client's name. If the form has a Full name field, the client's name is filled in from it.

  • If they're filling it in for the client, they enter their own name and choose their relationship to the client:

    • Parent

    • Guardian

    • Referrer

    • Practice staff

    • Other

See who submitted a form

To see who submitted a form:

  1. Go to the Clients tab.

  2. Open the client's profile and go to Forms.

  3. Find the completed form and click Actions, then Change log.

For each submission, the change log shows:

  • the date and time, in your time zone

  • who submitted the form

  • who they submitted it for and their relationship to the client

  • how it was submitted: emailed link, SMS link, copied link or embeddable form

The change log also records other activity on the form, such as when it was sent, emailed or texted, when a reminder went out, and who copied or opened the form link.

Things to note

  • The declaration is what the person filling in the form tells you. splose doesn't verify their identity.

  • The answer is saved in the change log only. It doesn't appear on the completed form or its PDF.

  • Forms submitted before this feature was released, or while the step was off, don't show who submitted them. Older submissions are labelled as submitted before attribution was available.

  • You can't turn the step on for all forms at once. Change it on each form template that needs it.

  • The step is part of the form's structure. You can't move or edit it in the form builder, and conditional logic can't hide it.

FAQs

Our forms already ask who is completing them. Do we still need this?

A question you add to a form is saved as one of the form's answers, which can be edited. The Before you submit step is recorded by splose in a change log that can't be edited, so it gives you stronger evidence in an audit. If you turn the step on, you can remove your own question from that form so clients don't answer twice. If a form doesn't need it, turn the step off.

Why is Before you submit showing in the form editor when the setting is off?

Refresh the page with Ctrl + Shift + R (Windows) or Cmd + Shift + R (Mac). The section only shows in the editor when the setting is on.

Can the change log be edited or deleted?

No. Submissions are recorded in a history that can't be changed.
