Custom fields

Learn how to create custom fields for your client records, and customise your forms to automate data entry.

Custom fields let you capture information that doesn't fit into splose's default fields. You can store this information on the client record and collect it automatically through your online forms, so data clients enter on a form populates their record without re-keying.

This guide covers the custom field types available, how to create a custom field, how it appears on the client record, and how to add custom fields to online forms.

Before you begin

Custom field types

splose offers several custom field types:

  • Short text: Short entries like names, IDs, or codes.

  • Long text: Longer entries like descriptions, feedback, or open-ended answers.

  • Multiple choice: Lets clients select one option from a list.

  • Dropdown: A predefined list of options. Set Selection Mode to single or multiple to control whether clients can choose one option or several.

  • Numerical: Numerical data like quantities, amounts, or scores.

  • Date picker: Specific dates or timestamps.

Why Use Custom Fields?

There are many reasons to leverage custom fields in your splose forms:

  • Automate Data Collection: Add custom fields into your online forms and have the data collected in custom fields automatically populate into the client record.

  • Gather Specific Data: Collect information critical to your business processes that isn't covered by the default splose fields.

  • Improve Data Accuracy: Ensure clients enter data in the correct format by specifying field types.

  • Export Data for Enhanced Reporting: Custom fields allow you to generate more comprehensive reports tailored to your specific data.

  • Streamlined Workflows: By collecting the data you need upfront, you can automate tasks and save valuable time.

Using the "Client Details" Section

Questions placed under the "client details" section of a form directly update the client profile. For example, if the name field is included in this section, the client profile name will be updated based on the submitted response. Use this feature intentionally when you want form responses to overwrite existing client profile information.

Custom fields with Display in client details enabled appear on the client details page and are available for use.

Once saved, they show in view mode on the record.

Add a custom field to an online form

The steps below outline how to place a custom field on a form so its responses flow into the client record.

  1. Go to Settings → Automation → Forms.

  2. Create a new form, or edit an existing one.

  3. Drag and drop the custom field type you want onto the form.

  4. Configure its settings, such as the label and whether it's a required field.

  5. Click Save.

Note: Embeddable intake forms do not update existing client profiles. Use a standard form sent from the client's profile instead.

Things to Note

  • When a client submits a form containing custom fields, that data automatically populates their client record — no repeat data entry.

  • The form builder uses a drag-and-drop layout, so you can see how the form will look as you add fields.

  • Embeddable intake forms do not update existing client profiles; use a standard form sent from the client's profile instead.

  • Questions in the "client details" section of a form directly overwrite corresponding client profile fields. Use this feature intentionally to avoid unintentional overwrites.

Troubleshooting and FAQs

Custom field data

Where is custom field data stored?

On the client record. Fields with Display in client details enabled appear on the client details page.

Do I have to enter custom field data manually?

No. If the field is added to an online form, the client's answer populates their record automatically when they submit the form.

Using and moving custom field details

Can I move the information I've stored in the "other details" field into custom fields in bulk?

Not automatically. Free-text in the other details field can't be split out and mapped into custom fields for you; the fields are structured and "other details" is unstructured, so there's no bulk migration. Set up the custom fields you need first, then move the information across per client as you work through them.

How can I pre-fill client details when sending a form?

To pre-fill a client's details when sending a form:

  1. Open the client's profile.

  2. Go to Forms → New form.

  3. Select the desired form template.

  4. Tick the Pre-fill client details option. How can I add a diagnosis to a client profile via a form? To include a diagnosis in a client profile:

  5. Create a custom field for the diagnosis under Settings → Business → Custom Fields.

  6. Enable the Display in client details option.

  7. Add the custom field to the intake form template. When a client submits the form, their response will populate the client record. Note that embeddable intake forms do not update existing client profiles; use a standard form sent from the client's profile instead.

My letter template shows "[No custom field specified]"; why aren't the form answers pulling through?

A form question only saves to a client's record if it's built with the custom-field block (you'll see the custom field name on the block).

If the question was added as a plain paragraph/text block, the answer isn't stored in a custom field, so letter templates have nothing to pull. Rebuild the question using the custom-field block and re-send the form.
