Create Embeddable Referral Form templates
Embeddable Referral Forms allows you to customise and embed your referral forms on your website. The Embeddable Referral Form connects to the Waitlist Screener to streamline client intake to automate and streamline administrative components of your client intake process. You can have more than one referral form if required to support unique workflows that you may have.

Create an Embeddable Referral Form template
Go to Settings, then Forms and click + New template.
Then select New embeddable form to start customising your form.
Build your form with sections and questions
Once you have selected the template, you can start by giving it a name, such as 'Referral form'.
Unlike standard online forms in Splose, you will not see the title of the Embeddable Referral Form in your browser. This is to give you flexibility in adding the title and description above the referral form on your website.
The left side includes the sections and questions that can be dragged onto the form.
The client details sections will populate these client data in the Splose client profile. A referral form will always create a new client, not look for an existing client and override the details.
Note that to create a client in Splose, the name is required, and therefore, it can't be deleted from an embeddable form.
Workflow automation
Set required questions
You can make certain questions required, which ensures that clients cannot submit the form until a response to the question is provided. You can do this by toggling the required switch in the bottom right of each section.
📝 Note: All question types can be duplicated except for personal details questions that update the client details section in Splose. That's because the client can only enter their personal details on a single form once (otherwise we wouldn't know which response to update!).
Duplicate questions
If you plan on asking similar questions, then you can use the duplicate option to duplicate questions, saving you time! This button is located in the bottom right, next to the required questions switch.
Delete questions
You can easily delete questions, and we always prompt a confirmation in case you accidentally click the delete button. This is also located in the bottom right, next to the duplicate button.
Drag and drop questions
While creating your template, you can drag and drop questions to change the position within any section and change the position of responses in certain question types.
Create your form
When you're ready and happy with your template, click Create.
Settings
Theme colour
This changes the colour of the section heading backgrounds and the submit button.
Header image
If you upload a header image, it will only be shown on the shareable link, and not the embeddable form version since this will be displayed on a website and has a better design without the logo.
Form completion message
You can add a brief form completion message which will show once the form has been completed.
Send client a PDF copy
You enable an automated email with a copy of the referral to the client if the client details email field is filled out in the form.

Share & Automate
On your embeddable form, click Share & Automate.
The Share and Automate settings will appear.
Publish form
Enabling this allows your referral form to be filled out by people outside of your workspace. You must have this enabled for the referral form to work on your website.
Shareable link
You can share the link and this can be useful in email templates or marketing initiatives.
Embed code
This is the code that you will use to show your referral form on your website.
Publishing a form on your website allows clients to easily complete the form, keeping in the same branding theme as your business.
Automation - waitlist screener
It is recommended to use the Waitlist Screener to help triage new referrals.
When a form is completed, Splose can either:
Create a new client in your workspace
Create a new client and add them to the waitlist screener
If you do not use the waitlist screener, ensure that you have the “send email to” setting active below so you are notified that a referral has been submitted.
Assign client tags
You can automate assigning client tags to new referrals once a form is completed.
Assign waitlist tags
You are able to automate assigning waitlist tags to new referrals.
Send email to
You can send an email to Splose users and/or groups.

Waitlist screener emails
If you're planning to triage new client referrals with the Waitlist Screener, you can assign customisable acceptance and rejection email templates to your embeddable form.

Waitlist Screener
The Waitlist Screener allows you to screen referrals submitted via your embeddable referral form or via the shareable link.
Remember, you must have the automation enabled in the Embeddable Referral Form settings to “Create client and add to the waitlist screener”.
We also recommend that you send an email to your intake team or email to as part of a workflow to indicate to your team that a new referral has been submitted. You can set up your administration or intake email as a Splose user to facilitate this workflow.
Waitlist Screener Rejection and Acceptance Emails
You can set up Waitlist Screener rejection and acceptance email templates in settings. Two default templates are set up for you and you are welcome to edit them.
To turn the Waitlist Screener email automation on, you need to select your default templates in the embeddable referral form share and automate settings. If you have more than one template, you are able to change the selection at the time of email however you must select a default in the share and automate settings.
Screener
Once a form is completed, go to the Waitlist tab, then click Screener.
You can click "Yes" to accept a referral, or "No" to reject a referral.
Either way, you will have the option of sending an email as part of your intake workflow.
You can use your default template, select from a range of templates you have created, write a personalised email or choose not to send an email.
The Waitlist Screener workflow is customisable since every business' intake workflow is unique and rejected referrals need to be handled with care.
Restoring a rejected referral
If you need to restore a rejected referral, click on the rejected tab within the waitlist screener.
Click on actions next to the referral you wish to restore and click “restore”. The referral will now be showing in the triage section of the waitlist screener again.
